Recital Commitment Acknowledgement
By checking the acknowledgment box below, I confirm that I have read and understand the following Recital policies and expectations. I also acknowledge that I have marked down the full recital date range and understand that specific dates and times will be released closer to the event

Why Participate in the Recital?
Participating in the recital is a cherished experience for dancers and families alike. It celebrates a year of growth, hard work, and creativity. Recital participation fosters teamwork, discipline, and lasting friendships—and is a commitment not only to Dance Dimensions, but also to your dancer’s classmates, teachers, and themselves.

Class Attendance Policy
  • Choreography is designed for the specific group of students in each class.
  • ​Attendance is critical. Students who miss more than 5 classes between March 1 and the recital may not be able to perform.
  • Private lessons do not replace missed class time during recital season, as they cannot replicate formation work or group choreography.
  • ​Missed recital classes may only be made up in non-recital classes, if available.

Recital Etiquette & Placement
  • Participation in the recital is based on desire, not ability. Every student is welcome to join if they are enrolled in a recital class and attend regularly and consistently.
  • ​Dancer placement in formations is at the discretion of the choreographer and will not be changed upon request.
  • It is inappropriate to ask for your dancer to be moved in formations.

Recital Day Security
  • For safety reasons, dancers may not be signed out early during the recital.
  • ​I trust Dance Dimensions to care for my dancer during the dress rehearsal and show and will follow proper dismissal procedures.

Fees & CommitmentRecital Participation & Rehearsal Fee:
  • $50 per student / $40 per sibling
  • Participation is confirmed by selecting “Yes” in the In Recital: Yes/No? box under student details during registration.

Costume Fee:
  • $85 per class/costume
  • ​The costume fee includes only the costume itself. It does not include alterations, shoes, tights, or any extra accessories your dancer may need for the show.

Billing Dates:
  • Recital participation fee: Week of January 12th
  • ​Costume fees: Week of February 9th

Refund Policy:
  • Recital fees and costume fees are non-refundable.
  • If the recital is canceled due to circumstances beyond our control, you will still receive the costume (no refund).
  • ​If you withdraw from the recital after costumes are ordered, you will still receive the costume (no refund).

Costume Measurements & Pick-Up:
  • Students will be measured for costumes in January/February. Please ensure your dancer is present for class during that time.
  • ​If your dancer is absent, you must bring them to the front desk for measuring as soon as possible.
  • Costumes will be sent home in the spring after full payment is received and once they come in. Please try them on immediately and notify us right away of any major sizing issues.
  • ​Alterations, if needed, are the family’s responsibility.

Arrival Time Expectations for Recital Day:
  • Dancers must arrive at their scheduled call time with hair and makeup complete.
  • ​Call times and show-specific instructions will be emailed closer to the recital.

Hair & Makeup:
  • Hair and makeup guidelines will be shared in advance and may vary by class or age group.
  • ​These requirements help ensure dancers are well-lit and performance-ready on stage.

Parent Volunteers:
  • We rely on parent volunteers to assist with backstage duties, dressing rooms, and check-in/check-out.
  • ​A volunteer sign-up form will be shared closer to recital season for those interested in helping.

Ticket Sales:
  • Tickets will go on sale closer to the recital. All audience members, including siblings, must have a ticket to enter.
  • ​We offer multi-show ticket discounts for families with dancers performing in more than one show. Please contact the front desk for details.

Photo/Video Policy:
  • Flash photography is strictly prohibited for the safety of performers.
  • ​A professional recital video will be available for purchase closer to the show.

Age Groups & Multi-Show Participation:
We group classes by traditional grade/age breakdowns, including:
2 yr olds
3 yr olds
4 yr olds
Kindergarten–Grade 1
Grade 2–3
Grade 4–5
Grade 6–8
​Grade 9–12
If your dancer is enrolled in multiple classes that span different age groups, they will likely perform in multiple shows. Similarly, siblings in different age groupings will most likely perform in different shows.
We offer ticket discounts for families attending multiple shows — please contact us for more information.

Communication & Email Acknowledgement
All recital information will be communicated via email. By acknowledging these policies, I agree to stay up to date with all emailed recital communication and ensure that at least one guardian remains on the studio email list throughout the season to receive all updates, instructions, and reminders.

By agreeing to this policy, I acknowledge that I have read and understood the policies outlined above.

studio policies
DDPAC STUDIO POLICIES 2025–2026 SEASON
By checking the acknowledgment box below, I confirm that I have read and understand the following studio-wide policies and expectations.

General Acknowledgements
  • I acknowledge that I have read and understand the studio policies outlined below.
  • I have reviewed the studio calendar and documented all important dates, including studio closures and recital dates.
  • I understand that all studio information will be communicated via email, and I agree to stay up to date with all studio messages. At least one guardian will remain subscribed to the studio email list for the duration of the season.

Age/Grade Groupings & Honesty Policy
  • All classes are divided by age or school grade:
    • If a class is labeled by age, the age cutoff is September 1, 2025.
    • If a class is labeled by grade, the dancer must be enrolled in that school grade for the 2025–2026 academic year 
  • There are no exceptions to age/grade requirements.
  • I agree not to falsify my dancer’s age or grade. If I do, DDPAC has the right to remove my dancer from the class.

Registration, Placement & Drop Policy
  • Students are evaluated during registration and the first few weeks of class to ensure proper placement.
  • Placement is based on age, grade, and ability and is determined solely by Dance Dimensions staff.
  • DDPAC reserves the right to adjust class placements at any time for the dancer’s benefit.
  • To drop a class, families must submit the Drop Class Form, located on our website under About > FAQ > “How do I cancel a class or drop out of the studio?”
  • We require a minimum of 30 days’ written notice with the drop class form prior to the next billing cycle to process a drop.
    • Example: A drop request submitted on November 15 will take effect on December 31 
  • Tuition is billed on the 1st of each month.
  • No verbal or emailed notice to teachers or the front desk will be accepted.
  • We do not automatically drop dancers for non-attendance—it is the family’s responsibility to notify us.

Make-Up Class Policy
  • Make-up classes must be taken within 30 days of the missed class.
  • Make-up classes may be taken in any available class of any style, with the exception of Acro and Aerial. To make up an Acro or Aerial class, the make-up must be taken in the same style, and only if the student originally missed an Acro or Aerial class. All make-ups must be taken in classes that are appropriate for the dancer’s age and level.
  • Make-ups do not roll over to other programs (e.g. summer).
  • Make-ups may not be available during recital season due to choreography in progress.
  • Missed recital classes during recital season (March-June) may only be made up in non-recital classes, if available.

Waitlist Policy
  • If a class is full, your dancer may be placed on a waitlist.
  • You will be notified if a spot becomes available.
  • We recommend enrolling in an open class in the meantime.

Trial & Drop-In Class Policy
  • Trial and drop-in classes are only available for open classes (not full or closed classes).
  • You must call the front desk the day before the class to check availability.
  • Do not register online for a trial or drop-in.
  • If space is available, you may attend by paying at the front desk right before class and receiving a class slip to present to the instructor.
  • Spots are not guaranteed or held in advance.

Class Cancellations & Schedule Changes
  • Dance Dimensions reserves the right to:
    • Change class schedules or instructors
    • Cancel or consolidate classes if necessary 
  • Always refer to our website for the most up-to-date class information.

Observer, Parent, and Studio Etiquette
  • Guardians and observers must watch from the hallway and may not sit inside the classroom unless explicitly invited by the instructor.
    • Please do not request to observe from inside the room 
  • If you have a concern or question regarding a class, please speak to the front desk.
    • Do not approach the teacher before, during, or after class 
  • Respectful behavior is expected at all times from both students and guardians.
    • Bullying, harassment, or disrespect toward teachers, staff, or fellow dancers will not be tolerated.
    • DDPAC reserves the right to dismiss any student or guardian for inappropriate behavior.

Dress Code, Cleanliness & Personal Belongings
  • Dancers must adhere to the dress code posted on our website.
  • Proper attire can be purchased at DDPAC. Students not in dress code may be asked to leave class.
  • Dress code is essential for technique evaluation, professionalism, and discipline.
  • Dancers must clean up after themselves at all times—this includes personal items, trash, and shared spaces.
  • No food, gum, or drinks are allowed inside studios (except water).
    • Pack nut-free snacks and a water bottle for long dance days. DDPAC does not have a drinking fountain—only a vending machine—so plan accordingly 
  • Dance Dimensions is not responsible for lost or stolen items.
    • Please do not bring valuables to the studio.
    • DDPAC reserves the right to check personal belongings if deemed necessary for safety.

Parking Lot & Pick-Up/Drop-Off Guidelines
  • Arrive on time to avoid congestion.
  • Drop dancers off 5 minutes before class begins and pick up 5 minutes before class ends.
  • Park only in authorized spaces and avoid blocking driveways or cars.
  • Additional parking is available on the other side of the building, in the alley, on Ventura, and in the community behind the studio.

Health & Safety
  • Families must notify DDPAC of any medical conditions, allergies, or special needs. This information will remain confidential and used only for safety purposes.
  • Drugs, alcohol, vaping, theft, or any illegal substances are strictly prohibited. Violation of this policy will result in immediate dismissal from the studio.
  • If DDPAC suspects involvement or receives reports of inappropriate behavior, we reserve the right to question and investigate accordingly.

Code of Conduct
  • DDPAC reserves the right to refuse service or remove families from classes or the studio for:
    • Abusive or disruptive behavior
    • Violations of studio policies

Financial Policies
  • Continuous non-payment may result in removal from classes or restriction from participating in studio events.
  • Dancers with an outstanding balance will not be permitted to participate in recital or other studio events.
  • Families with unpaid balances may not register for optional paid events until the balance is cleared.
  • If DDPAC must cancel classes or events due to circumstances outside our control, refunds or credits will be issued at the studio’s discretion.
PAYMENT POLICIES
Financial Responsibility Acknowledgment
By checking the box below, I acknowledge that I have read and understand the following financial policies and accept full responsibility for all tuition and fees associated with my dancer’s participation at Dance Dimensions Performing Arts Center.

Tuition & Payment Structure
  • Dance Dimensions operates on a “school year” (yearly) program, running from September through June.
  • Tuition is based on a yearly fee, divided evenly into 10 monthly installments for your convenience.
  • Monthly tuition is not based on the number of classes per month and is not adjusted for holidays or closures. Some months have four classes, others have five.
  • Initial payment is charged:
    • Upon registration or before the season begins, depending on when you register.
    • If you register before the season begins, your registration fee and first month’s tuition are charged at sign-up.
    • You will not be charged again on September 1, and monthly billing will begin October 1 
  • If you register mid-year, your tuition will still follow the prorated yearly schedule, and monthly payments continue through your final month of enrollment.
  • All students pay full monthly tuition from their first month through their last, including December and June.

Registration Fees
  • $40 per student / $25 per sibling
  • Applies once per dancer per school-year session (September–June), regardless of when registration occurs.
  • Registration fees are non-refundable and non-transferable.

Payment Method & Policies
  • Monthly payments are processed automatically via credit or debit card (Visa, MasterCard, or AMEX only).
  • Automatic billing occurs on the 1st of each month.
  • If your card is declined, you will be notified via email.
  • A $25 late fee will be applied if payment is not resolved within 7 days.
  • If payment is still not received after 2 weeks, your dancer will be removed from class, and we will not save their spot.
    • Re-enrollment is only possible if space is still available and the full balance is paid.
  • Tuition is non-refundable.
    • There are no credits, refunds, or transfers for missed classes, absences, or vacations.
    • Accounts cannot be placed “on hold” for any reason.
    • Please do not ask for prorating or exceptions—make-up classes may be available (see Make-Up Policies).
    
Recital Participation & Additional CostsIf you choose to participate in recital, the following required fees apply:
  • Recital Registration Fee:
    • $50 per dancer / $40 per sibling
    • Charged the week of January 12th 
  • Costume Fee:
    • $85 per class/costume
    • Charged the week of February 9th
    • Costume fee does not include alterations, shoes, tights, or additional accessories 
Note: If you are not paid in full at the time of recital-related deadlines, you will not receive costumes, tickets, or be permitted to perform in the recital.

Recital/Event Eligibility
  • All balances must be paid in full for your dancer to:
    • Participate in the recital
    • Receive costumes or tickets
    • Register for optional paid events
    
Withdrawing & Re-Enrollment
  • If you stop monthly tuition payments or end your “yearly tuition student” status for any reason:
    • You will be considered a new student if returning during the same session (Sept–June).
    • A new registration fee will be required.
    • Class placement is not guaranteed; your spot may be taken.
    
Split Family Payment Arrangements
  • We can accommodate split payments between two individuals (e.g., two parents, or a parent and grandparent).
  • Both credit cards must be provided before the first billing cycle begins.
  • DDPAC cannot act as a mediator between separated or non-communicating guardians.
    • The person registering the dancer online is responsible for entering a valid credit card.
    • This card will be charged unless an alternative is provided.
    
Sibling Discount
  • A 5% discount is available for siblings.
  • The discount applies to the dancer taking fewer hours per week.

Program Disruptions
  • If DDPAC must cancel classes or events due to circumstances outside our control, credits will be issued at the studio’s discretion.​
ATTENDANCE & ABSENCE POLICIES
Attendance & Absence Policy
By checking the box below, I acknowledge that I have read and understand the attendance and absence policies outlined below.

Attendance Expectations
  • Consistent attendance is essential for student growth, confidence, and class progression.
  • Inconsistent attendance can hinder your dancer’s development and impact the overall progress of the class.
  • Attendance is particularly important in the Spring months leading up to the recital, when choreography is being taught, cleaned, and finalized.
  • Excessive absences may result in your dancer being removed from recital choreography, at the sole discretion of DDPAC teachers and management.
  • If your dancer is removed due to poor attendance, tuition, costume fees, and recital registration fees will not be refunded.

Make-Up Classes for Absences
  • Make-up classes are available and must be taken within 30 days of the missed class.
  • Make-ups are not guaranteed, especially during recital season when classes focus on specific choreography and formations.
  • Make-up classes do not roll over into other programs, such as summer.
  • Dancers may take a make-up class in any available class that is appropriate for their age and level, except for Acro and Aerial:
    • To make up an Acro or Aerial class, your dancer must have missed the same style.
    • Acro can only be made up in Acro; Aerial in Aerial.

Absences, Vacations & Financial Responsibility
  • Tuition is not prorated for missed classes, vacations, or absences.
  • Accounts cannot be placed “on hold” for any reason.
  • Attendance issues do not exempt families from monthly payments or registration fees.
  • Please refer to the Make-Up Policy above for options.
MAKE UP POLICIES
Make-Up Policy
By checking the box below, I acknowledge that I have read and understand the make-up class policies outlined below.

Eligibility for Make-Up Classes
  • If a dancer misses a regularly scheduled class, they are eligible to schedule a make-up class.
  • Make-ups are not available for days when the studio is closed for holidays or breaks.
  • No credits or refunds will be issued for missed classes under any circumstance.

Scheduling & Timeframe
  • It is the family’s responsibility to schedule make-up classes through the Parent Portal.
  • Make-ups must be taken within 30 days of the absence.
    • After 30 days, the make-up expires and cannot be reinstated or rolled over.
    • Make-ups do not transfer to summer sessions or future program years 
  • Make-ups can only be taken if the dancer is actively enrolled and paid for the month during which the make-up is scheduled.
    • If a student drops from the studio, any unused make-up classes are forfeited.
Make-Up Class Restrictions
  • Dancers may take a make-up class in any style, as long as it is appropriate for their age/grade and level.
  • However:
    • Aerial and Acro classes may only be made up in the same style, and only if the dancer is currently enrolled in that style.
    • Dancers not enrolled in Aerial or Acro may not take those classes as make-ups.
Recital Season Limitations (March–June)
  • Make-up classes become more difficult to schedule during recital season (March through June).
  • As recital approaches, many classes are focused on choreography, formations, and blocking, making participation in make-ups disruptive and less effective.
  • Missed recital classes may only be made up in non-recital classes, if available.
  • Families are strongly encouraged to avoid absences during this time if their dancer is participating in the recital.
RELEASE & PERMISSIONS
Release & Permission Policy
By checking the box below, I acknowledge that I have read and understand the liability and release policies outlined below.

General Liability Waiver
I understand that participation in dance classes, rehearsals, performances, and related studio activities may involve the risk of injury, illness, or other harm. By enrolling at Dance Dimensions Performing Arts Center (DDPAC), I voluntarily assume all risks associated with participation for myself and/or my child.

I hereby release and hold harmless Dance Dimensions Performing Arts Center, including its owners, directors, instructors, employees, contractors, agents, volunteers, successors, and all class or performance locations, from any and all liability, claims, demands, or causes of action resulting from participation in any DDPAC activity.

This release applies to all injuries, illnesses, damages, or losses that may be sustained before, during, or after participation in classes, events, performances, private lessons, or studio-sponsored activities, whether occurring on or off studio premises.

Assumption of Risk & Indemnification
I understand and acknowledge the inherent risks of physical activity, including dance, and assume full responsibility for any personal injuries, accidents, illnesses, property damage, loss, or even death that may occur as a result of participation.

I agree to indemnify, defend, and hold harmless Dance Dimensions and its representatives from any and all claims, suits, costs, and expenses—including attorney fees—arising from or related to my or my child’s involvement in any studio-related activity.

Force Majeure
I understand that Dance Dimensions Performing Arts Center shall not be held liable for any failure or delay in performance arising from causes beyond its reasonable control, including but not limited to acts of God, natural disasters, government restrictions, pandemics, fire, flood, labor disputes, or other unexpected interruptions.

In such cases, I understand that DDPAC reserves the right to modify, reschedule, or cancel classes or events as necessary, and that any refunds or credits will be determined solely at the studio’s discretion.
WAIVER OF LIABILITY
Waiver of Liability
By checking the box below, I acknowledge that I have read and understand the waiver of liability outlined below.

Participation & Assumption of Risk
I, and/or my minor child, wish to participate in a dance class, rehearsal, workshop, dance party, event, or any other activity at Dance Dimensions Performing Arts Center (DDPAC). This includes activities offered through official DDPAC programming as well as participation in a private studio rental or event held on DDPAC property.

I understand that dance and physical movement involve inherent risks of injury, including but not limited to muscle strain, sprains, falls, and other physical harm. I acknowledge that these risks may arise from my own actions or inactions, the actions of others, or the condition of the premises.

Medical Responsibility & Fitness to Participate
I understand that it is my sole responsibility to consult with a physician prior to participating in any activity at DDPAC. I certify that I am (or my child is) physically fit and in proper health to safely participate. I affirm that I have no medical condition that would prevent full participation in dance or movement-based activities.

If my dancer is unable to participate in class due to injury or illness, I agree to provide a doctor’s note before returning to class.

Emergency Medical Consent
In the event of a medical emergency, I authorize DDPAC staff to obtain emergency medical care for my child if I cannot be reached in time. I accept full financial responsibility for any medical treatment or transportation incurred on my child’s behalf.

Waiver & Release of Liability
I, on behalf of myself, my minor child(ren), and our respective heirs, executors, administrators, successors, and assigns, hereby waive, release, and discharge DDPAC and its owners, instructors, employees, agents, contractors, and representatives from any and all liability, claims, demands, or causes of action related to injuries, damages, or losses—whether physical, emotional, or financial—sustained as a result of participation in any activity at or affiliated with DDPAC.

This includes injuries or damages arising from the negligence or acts of DDPAC or its representatives, to the fullest extent permitted by law.

Indemnification
I agree to indemnify and hold harmless DDPAC, its owners, employees, instructors, contractors, and representatives from any and all claims, damages, and expenses—including attorney fees—arising from my or my child’s participation in any studio activity.

Property Loss or Damage
DDPAC is not responsible for lost, stolen, or damaged personal property, including but not limited to:
  • Cell phones
  • Clothing
  • Shoes
  • Jewelry
  • Cash or credit cards
  • Bags or other belongings
This applies to property brought to or left on the premises, including the lobby, dressing rooms, studios, parking lot, or any affiliated performance or rehearsal location.

Acknowledgment of Responsibility
I understand and accept full responsibility for any injuries, losses, or damages—known or unknown—that may result from participation in any DDPAC activity. I voluntarily enter into this agreement with full understanding of its terms.

Policy Updates
I understand that DDPAC may update its policies or this waiver as needed. It is my responsibility to review any future changes communicated by the studio.
CREDIT CARD & EMAIL POLICY
Credit Card & Email Policy
By checking the box below, I acknowledge that I have read and understand the credit card and email communication policies outlined below.

Credit Card Requirement
  • A valid credit or debit card (Visa, MasterCard, or AMEX only) is required to be on file for all student accounts.
  • Tuition is billed on a monthly auto-pay system, with charges processed on the 1st of each month.
  • If you wish to pay by cash or a different credit/debit card, payment must be made before the 1st of the month to avoid automatic charges to the card on file.
  • No checks accepted.
  • The initial charge is processed immediately upon registration in order to reserve your spot in class.
  • If your card is declined and payment is not resolved, your dancer’s spot will not be held, and enrollment will only be confirmed if space is still available at the time payment is received.

Email Communication Requirement
  • A valid email address is required for all families.
  • Email is our primary method of communication for important updates, studio announcements, billing, policies, class changes, and recital information.
  • It is your responsibility to ensure that you are receiving studio emails.
    • If you are not receiving messages, please notify the front desk immediately so we can assist in resolving the issue.
INFECTIOUS DISEASE & COVID-19 POLICIES
Infectious Disease & COVID-19 Policy
By checking the box below, I acknowledge that I have read and understand the infectious disease and COVID-19 policies outlined below.

Health Risk & Assumption of Responsibility
Dance Dimensions Performing Arts Center (DDPAC) is committed to the health and safety of its students, families, and staff. However, by attending in-person classes, rehearsals, events, or performances, I understand that participation carries inherent risks of exposure to contagious illnesses, including but not limited to COVID-19, influenza, RSV, or other viruses.

I voluntarily assume all risks related to exposure, infection, and any resulting complications for myself or my child(ren) and agree to hold Dance Dimensions Performing Arts Center, its employees, contractors, volunteers, and affiliates harmless from any liability arising from participation during a pandemic, epidemic, or general public health concern.

Symptom Awareness & Health Screening
Common symptoms of contagious illness, including COVID-19, may include:
  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • Sore throat
  • Congestion or runny nose
  • New loss of taste or smell
  • Nausea or vomiting
  • Diarrhea
This list is not exhaustive. I agree to monitor for symptoms and keep my dancer home if they show signs of illness.

Family Commitments
By attending in-person programming at DDPAC, I agree to the following:
  • I will monitor my dancer’s health and not send them to class if they are sick or have symptoms of illness.
  • If my dancer tests positive for COVID-19 or another contagious illness, I will notify the studio and follow any applicable return-to-class policies.
  • I will not bring my dancer to class if they or a household member has had a known exposure to a contagious illness and are required to isolate per public health guidelines.
  • I acknowledge that DDPAC cannot be held liable for any exposure to illness due to misinformation or health history not disclosed.

Studio Safety Measures
DDPAC staff and instructors will:
  • Clean and disinfect high-touch surfaces regularly.
  • Provide hand sanitizer at the front desk.
  • Follow Los Angeles County public health guidance and best practices during heightened risk periods.

We ask all families to:
  • Use hand sanitizer and practice good hygiene.
  • Keep dancers home when sick or recovering.
  • Bring a labeled water bottle to class.

Virtual Instruction & Program Disruption Policy

If DDPAC must adjust or suspend in-person programming due to public health guidelines, emergency orders, or illness-related risk:
  • Classes may transition to a virtual format without refund.
  • The 30-day drop policy will still apply.
  • No credits or refunds will be given for the switch to online learning.

Photography & Videography Release
By checking the box below, I acknowledge that I have read and understand the photography and videography release policy outlined below.

Media Use Permission
I grant Dance Dimensions Performing Arts Center (DDPAC), along with its representatives, employees, contractors, and assigns, the right to photograph and/or record video or audio of my child(ren) in connection with studio activities and events.

I authorize DDPAC to use, reproduce, copyright, publish, and distribute these materials—whether in print, online, or other digital formats—for lawful purposes including but not limited to:
  • Publicity and promotion
  • Social media content
  • Advertising and marketing
  • Studio website and printed materials
  • Internal and external communications
This release applies with or without the use of my child’s name, and extends to any edits or modifications made to the original media.

Rights & Compensation
I understand that:
  • I am assigning all rights to these photographs, videos, and recordings to DDPAC.
  • I will not receive compensation for the use of any media.
  • DDPAC retains the right to exhibit, broadcast, or distribute this content without limitation.

Opt-Out Option
If you do not grant permission for your dancer to be photographed or recorded, please email the studio directly at [email protected] to opt out of this release.
Written notice must be submitted at the time of registration in order to be honored.
