Participation in recital is an agreement to be charged the Recital fee of $50/$40 sibling and also an agreement to be charged costume fees $85 per costume/class. Families who select "Yes" to participate in the Recital will be charged the $50/$40 sibling Recital registration fee the week of January 13th. Costume fees will be charged the week of February 10th. If Recital needs to be canceled due to events out of our control, we will give you the costume (no refunds). Recital registration is non-refundable.

We build teamwork, discipline, life-long friendships and a passion for dance! The Recital is also a commitment to Dance Dimensions, to the instructor, fellow students and to yourself, the dancer.
Choreography of Recital dances is planned specifically for the number of students in class and for their individual, unique abilities. Therefore, students MUST attend classes in order to be included. Participation is based solely on desire, not ability. Every student may be in the recital who desires to participate and who has satisfied the payment, class participation, and other requirements as detailed in the Studio Policies. By answering "yes" in the recital participation box, I understand the importance of attending classes in order to participate in the Dance Dimensions Recital. If the dancer misses more than 5 classes between March 1st and the recital, he or she may not be able to participate in the recital. As a dancer or guardian, I acknowledge that I trust the teacher/choreographer in where my dancer is placed in formations, and I will not ask for the dancer to be moved under any circumstances.
I understand this is inappropriate to do so. By answering "yes" in the recital participation box, I agree that my credit card will be charged $50/$40 sibling per student the week of January 13th and $85 per costume/class the week of February 10th.

studio policies
I acknowledge that I am going to read the policies stated below so that I understand the studio policies.
I acknowledge I have seen the studio calendar and all the dates have been documented, including studio closures and recital.
All classes are divided by age or grade. If the class is by age, the age cutoff of September 3, 2024 (first day of our 2024-2025 session). There are no exceptions. If the class is by grade, the dancers must be in that school grade for the 2024-2025 season. I acknowledge that I will not lie about my date of birth, and if I do, DDPAC has the right to remove me from the class that does not fit my age group.
You must use the "DROP CLASS" form if you drop out of a class. We must have a minimum of 30-days written notice before the next billing cycle for a class to be dropped. We charge tuition on the 1st of the month. So for example, if you request to drop on November 15th, you will be dropped at the end of the following month (December 31st). The notice must be given prior to withdrawal in order to avoid the next month's billing cycle. NO email or verbal notice to the teacher or to the front office will be accepted. This form is available online and in the studio for your convenience. We do not automatically drop students for any reason (ie if they are absent many weeks in a row). It is up to you to keep us informed.
Students will be evaluated during the registration process and during the first few weeks of class to make sure they are in the right level. Placement is based solely on ability and age/grade. This is done at the discretion of the director and instructors. Dance Dimensions reserves the right to shift students into other classes if their ability does not match the level of the class, as determined solely by Dance Dimensions staff. All decisions are made with the dancers' best interests at heart!
Dance Dimensions reserves the right to change the schedule and/or cancel classes at any time. Classes are subject to change. Especially with newly scheduled classes, check the website for the most up-to-date information.
Dance Dimensions is not responsible for students who leave the facility without supervision. Please advise your dancer not to leave the premises without your permission.
Students must adhere to the Studio dress code as detailed on our website..Students may purchase appropriate attire at DDPAC, or will be asked to leave class and return dressed appropriately. DDPAC's dress code is in place for the teachers to evaluate technique, to provide a professional environment, and to instill discipline.
Respectful behavior by both guardians and students is expected at all times towards Dance Dimensions staff and fellow students. Bullying will never be tolerated. We reserve the right to dismiss a guardian or student who does not exhibit respectful behavior.
No food, drink, cell phones or gum allowed in the studios. Only water is allowed inside the studio. Make sure your dancer arrives with their own water and snacks if it's a long day (no nuts).
Dance Dimensions is not responsible for lost or stolen items. Please do not bring anything of value. DDPAC reserves the right to look through personal belongings if a situation arises where it becomes necessary.
I acknowledge that it is important to obey all rules pertaining to the parking lots. Please be aware of directions to enter and exit, do not block other cars and keep driveways open. Do not leave your car if it is not in an authorized space. Please arrive on time so you are not rushed. Drop your dancer 5 minutes before class starts and arrive 5 minutes before class ends. We must make sure that the flow of arrivals and departures works so that there is no congestion. Parking can be crowded at times, so please follow proper procedures for the safety of our children. There is plenty of parking in the alley and on the west side of the building.
Dance Dimensions reserves the right to dismiss or refuse classes to anyone who does not comply with the policies or who otherwise in our sole judgment disrupts the harmony and/or reputation of the studio. We also reserve the right to ban any family from the DDPAC property for continuous non-payments and/or abusive behavior. If for reasons out of our control DDPAC must cancel classes and/or events, refunds/credits will be decided by DDPAC based on the circumstances.

PAYMENT POLICIES

This is a "school year" ("yearly") program where on the first of the month, automatic monthly payments are scheduled for your convenience. Tuition is set up so that the yearly fee is prorated over the 10 months. The initial payment is charged upon sign up or before the start of the season, depending on when you register. If you register before the season, your card will be charged the registration fee and first month's tuition -- therefore, you will not be charged September 1st and your monthly payments will begin on October 1st.
If you join mid-year, you are still subject to this prorated year schedule. All students pay full tuition every month from their first month through their last, INCLUDING DECEMBER AND JUNE. Some months will have four weekly classes; some five. Holiday weeks, like those in late December where there are no classes, may be offset by other months that have five weeks of classes.
Studio Registration Fee: $40 per student, $25 sibling per Fall-Spring session (Sept-June). All payments are non-transferable and non-refundable and apply to this time period only. To be payable when registering into the studio no matter when during the session the registration occurs.
Monthly payments ONLY available by automatic Credit Card (Visa, MC, or AMEX only) or Debit Card charge. Automatic charges must go through on the first attempt. Tuition is non-refundable. There are no refunds, credits, or extensions given for missed classes. Dance Dimensions does not prorate for absences or vacation time. We do not put accounts “on hold.” If your dancer is asked to leave Dance Dimensions Performing Arts Center due to a break in the policy agreement, there will not be any refund or credit given.
BAD CARDS: You will receive an email if your credit card does not go through. Failure to process payment or supply a new credit card within 7 days will result in a $25 late fee. If there is still a balance past two weeks, my dancer(s) will not be allowed to continue her classes, as repeated credit card failures without proper payment in a timely manner jeopardizes participation in the studio. Tuition applies to every month of enrollment, September through June, for all registered students, regardless of vacation or absence. We do not prorate if you are absent or away on vacation. No adjustments, credits or transfers will be made for missed lessons. Please don't ask! We do not put accounts “on hold” for any reason. Make-up classes are available. See Make-Up Policies.

There is a 5% discount on tuition for siblings. The "Sibling," for discount purposes, is the child taking less hours weekly. We can accommodate a SPLIT FAMILY with two people paying for their dancer. We can charge two cards on a monthly basis, whether 2 parents or a parent and grandparent, etc. HOWEVER, we cannot be put in the middle of parents who no longer have an amicable relationship. It is not fair to ask us to pursue a parent for credit card information and permission to charge each month. The parent or guardian who registers the dancer online needs to put a credit card into the system during registration, like all our families, and this is the card that will be charged the full monthly amount. If the idea is to split the charge on two cards, please supply us with the information for the second card. Until that information is received, we will continue to charge the single card that is on file.

Make-ups can only be taken if the student is currently enrolled. In other words, make-ups can only be scheduled and taken if the student has paid for the month during which they are to be taken. If you drop out of the studio, you forfeit all make-up days. No credit or refunds given for missed classes.
Dancers can take make-up classes in their appropriate age/grade and level only. They may take a style different from the class they missed (except aerial and acro if they are not currently enrolled in those classes). Make-ups become difficult to schedule as recital season progresses (May and June) and may not be available as the shows get closer. Classes are practicing for the recital, and therefore it becomes difficult to do a make-up in a class that is practicing specific choreography for the recital. Please take note of this and do not miss during recital season, especially if you are participating in the show.

I hereby release Dance Dimensions and all class and performance locations, as well as their owners, agents, employees, volunteers, contractors and successors, from liability. I agree to indemnify them for and hold them harmless from losses, suits, claims and demands of every kind and character arising out of and in conjunction with my Child's or my own participation in these classes. I recognize and acknowledge there are certain risks of physical injury, and I agree to assume the full risk of personal injuries, accidents, illnesses, death, damages and/or property loss that I or my minor child may sustain as a result of participating in any activities connected with or associated with Dance Dimensions programs, classes or private lessons.
I hereby release Dance Dimensions and all class and performance locations, as well as their owners, agents, employees, volunteers, contractors and successors, from liability. I agree to indemnify them for and hold them harmless from losses, suits, claims and demands of every kind and character arising out of and in conjunction with my Child's or my own participation in these classes. I recognize and acknowledge there are certain risks of physical injury, and I agree to assume the full risk of personal injuries, accidents, illnesses, death, damages and/or property loss that I or my minor child may sustain as a result of participating in any activities connected with or associated with Dance Dimensions programs, classes or private lessons.
WAIVER OF LIABILITY
I acknowledge that I am going to read the policies stated below so that I understand the waiver of liability.
I (and/or my minor child) wish to participate in a dance class, rehearsal, dance party or other activity at Dance Dimensions Performing Arts Center (hereafter known as "DDPAC"), either as part of DDPAC's programming or as a participant of a private studio rental of DDPAC facilities. As a condition of DDPAC agreeing to allow my participation in such an activity, I hereby confirm and agree to the following:
I recognize that the activity of the type I intend to participate in at DDPAC requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my responsibility to consult a physician prior to and regarding my participation in any activity I attend at DDPAC. I represent that I am physically fit and have no medical condition that would prevent my full participation in any such activity. I understand that DDPAC is not liable for any injuries to any participant, dependent(s) or guest(s) or be subject to any claim or demand for injury or damages whatsoever, including, without limitation, those damages resulting from acts or negligence on the part of DDPAC, its owners, agents, instructors or employees. I, and on behalf of my executors, administrators, heirs, assignees and successors, do hereby knowingly and voluntarily forever waive, release, discharge and agree to indemnify and hold harmless DDPAC, its owners, instructors, employees, assignees and successors from all such claims, demands, injuries, damages, actions or causes of action for any injury or damage that I may sustain as a result of my participation in an activity anywhere within the confines of DDPAC or its performance/practice locations.
DDPAC shall not be responsible to me, my dependents or guests for property damaged, lost or stolen in or surrounding DDPAC confines, including but not limited to clothing, cell phones, cash, credit cards, jewelry and any and all personal property brought to and/or left at DDPAC.
I agree to take full responsibility for any risks, injuries, loss or damages known or unknown which I incur, or may incur, as a result of participation in any activity at DDPAC. If my dancer cannot participate due to injury or illness, I will provide DDPAC with a doctor's note to return.

DDPAC shall not be responsible to me, my dependents or guests for property damaged, lost or stolen in or surrounding DDPAC confines, including but not limited to clothing, cell phones, cash, credit cards, jewelry and any and all personal property brought to and/or left at DDPAC.
I agree to take full responsibility for any risks, injuries, loss or damages known or unknown which I incur, or may incur, as a result of participation in any activity at DDPAC. If my dancer cannot participate due to injury or illness, I will provide DDPAC with a doctor's note to return.

Dance Dimensions requires a valid credit card or debit card on file for all students, which is used for automatic monthly payments. If you wish to pay a different way besides the auto payment, you still need a working card on file, and must bring in alternate payment BEFORE THE FIRST OF THE MONTH. Your card on file will automatically be charged on the first of the month, unless prior arrangements are made. Credit cards or cash only.
INITIAL CHARGE is made immediately in order to reserve your space in class!

COVID-19 POLICIES
I acknowledge that I am going to read the policies stated below so that I understand the COVID-19 policies.
People with COVID-19 have had a wide range of symptoms reported – ranging from mild symptoms to severe illness. Symptoms may appear 2-14 days after exposure to the virus.
People with these symptoms may have COVID-19:
- Fever or chills - Cough - Shortness of breath or difficulty breathing - Fatigue - Muscle or body aches - Headache - New loss of taste or smell - Sore throat - Congestion or runny nose - Nausea or vomiting - Diarrhea This list does not include all possible symptoms.
Dance Dimensions Performing Arts Center (“DDPAC”) cannot guarantee that you or your child(ren) will not become infected with COVID-19. It is important to acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you and/your child(ren) may be exposed to (or infected by) COVID-19.
In understanding and agreeing to this policy, you are voluntarily agreeing to assume all of the foregoing risks and accept sole responsibility for any injury to your child(ren) or yourself (including COVID-19, illness, damage, loss, claim, liability, or expense of any kind), that you or your child(ren) may experience or incur in connection with your child(ren)’s attendance at DDPAC or participation in any dance programming. On your behalf, and on behalf of your children, you hereby release, covenant not to sue, discharge, and hold harmless DDPAC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.
By agreeing to the policy, you understand the following:

I affirm that I currently have not experienced the aforementioned symptoms within the past 30 days. Furthermore, I will immediately get tested if I do have any of the aforementioned symptoms.
I affirm that I have not been diagnosed with COVID-19 within the past 14 days. Furthermore, I will immediately discontinue classes if I am diagnosed with COVID-19.
I affirm that neither I, nor any member of my household, has traveled outside of the country or to any city considered to be a "hot spot" for COVID-19 infections within the past 14 days.
I understand that Dance Dimensions cannot be held liable for any exposure to the COVID-19 virus caused by any misinformation on this form or the health history provided by each Student.
Dance Dimensions will be adhering to the following enhanced procedures to prevent the spread of COVID-19.
OUR INSTRUCTORS AND STAFF WILL…
- Clean dance floors, bathrooms and frequently touched surfaces often.
- Wash/Sanitize our hands consistently.
- Provide hand sanitizers throughout the premises.
- Follow all LA County Guidelines.
WE ARE REQUESTING THAT EACH OF OUR STUDENTS, PLEASE…
- Utilize the hand sanitizer and wash hands consistently.
- Bring their own large bottle of drinking water, labeled.
- Follow all LA County Guidelines.
If DDPAC must switch to virtual classes, our 30-day drop policy still applies. DDPAC will not offer refunds for switching to virtual options.
